The hamstrings are a muscle group which are situated at the back of the thighs and are commonly involved in injury from vigorous activities and sports. Hamstring injuries are more commonly involved in areas of the muscle which are closer to the buttock and on the outside of the back of the thigh. There are three muscles in the hamstring group and they do not have common names, being termed semimembranosus, semitendinosus and biceps femoris. The latter is the muscle most commonly involved in typical sporting injuries.

Hamstring injuries are classified for ease of diagnosis and treatment into various grades of severity. The least serious injury with a number of damaged muscle fibres is a grade 1 injury, rated as a mild muscle strain. More serious involves a larger number of muscle fibres being damaged and a reduction of muscle strength which is obvious on testing and this is a grade 2 injury. In the most serious or grade 3 injury there is a rupture right through the substance of the tendon and muscle. Most injuries are located at the muscle and tendon junction and high up near the buttock, although the biceps femoris has a very long junction, most of its length.

The origin of the hamstring tendons is the bones in the buttock which we sit on, known formally as the ischial tuberosities. The tendons are attached to this bony area and a violent movement into hip bend with a large overall movement range can avulse (tear off) the tendon’s junction from its bony bed. Water skiing is a risky activity for this problem. Most hamstring injuries occur in younger people who overwhelmingly perform athletic activities, with typical sports for this injury being field sports, contact sports, track activities such as sprinting, football or rugby.

The hamstrings run down from their origin at the ischial tuberosity deep in the buttock along the posterior upper leg and insert into the upper tibial area. If the hamstrings are under eccentric contraction, i.e. they are both contracting under load and lengthening at the same time, they are particularly susceptible to a strain. Direct blows in contact sports can cause a contusion of the muscle and an avulsion fracture can occur when a water skier falls forward violently at the hip with their legs straight. Patients typically report hamstring injuries as coming on very suddenly and that they can clearly hear the muscle go pop.

When an injury occurs the patient reports an immediate pain in the back of the upper leg with the event occurring either when the patient has got tired late on in the activity or before they got warmed up in the beginning. In less severe injuries there might be minor interruptions in functional activities such as pain on going uphill or climbing stairs. Physiotherapy examination of the back of the leg will often reveal little but resisted knee flexion is often painful, where the physiotherapist pushes against the ankle as the knee is bending.

An increase in the risk of suffering from a hamstring injury is thought to occur if the quadriceps and hamstring strength ratios are incorrect, the person is tired, there is a poor warm up or a limitation of flexibility. A previous history of hamstring injury is a significant risk of having a recurrent strain. It will take a few minutes or more for the heat or cold to penetrate the clothing so start timing when you actually feel it. Remember twenty minutes at one to two hour intervals to avoid skin irritation or damage.

Many also believe laying on a heating pad will help with the pain and it does until you remove the heat. The heat will deaden pain receptors, once the heat is removed the receptors begin sending signals to brain, and the pain returns, worse than before the heat was applied, this is because heat causes inflammation to worsen if it present.

Taking short walks when sciatica is being caused by problems with discs in the lower back, can be more beneficial than sitting. Sitting positions compress the spine which will place more pressure on the sciatic nerve causing more pain. Inflammation and swelling of the sciatic nerve will occur with both. Lower Back Pain Causes and Treatments

Treatments for lower back pain vary depending on the cause. If you know the cause then you can find treatment options that will help relieve the pain. The First thing you should know is that excruciating lower back pain is commonly referred to as sciatica. Sciatica, or excruciating lower back pain, is caused by the irritation and inflammation, or compression of the spinal column, or both. The sciatic nerve, the longest nerve in the body is about the size of a nickel in diameter and starts at the lower back and goes all the way down to your toes.

As the nerve runs down the leg to the toes it branches off into smaller nerves all along the way, if at any point any part of the nerve becomes irritated, the lower back pain or sciatica will ensue. There are four major causes of sciatica as follows:
•    Nerve root compression
•    Herniated spinal discs
•    Spinal degeneration
•    Piriformis syndrome

The two most common being, Piriformis syndrome, and Nerve root compression. Piriformis syndrome, is when the Piriformis muscle is damaged and becomes inflamed, which causes swelling. The swollen muscle then causes pressure on the sciatic nerve, causing it to become inflamed thus, the lower back pain you are feeling. Nerve root compression occurs when the spinal column becomes compressed and pinches the roots of the sciatic nerve again, causing it to become inflamed.

In either case to get relief from the pain, (actually for any of the four things above) you should, when you first experience the pain, use ice packs to reduce the swelling that comes with the inflammation. If you use heat (as I did for many years) you will cause the inflammation to worsen which in turn, will causes more swelling and more pain. So, for the first 48 hours after first experiencing sciatica use an ice pack for twenty minutes once every two hours, this is to avoid skin damage, you should also place a cloth between the skin and ice pack as well.

Another thing you can do is backward extension exercises to help stretch the muscles that cause compression of the spinal cord. Some of the exercises, if done daily, can keep sciatica from returning, and they can be done in eight minutes, that is eight minutes a day for a pain free back.
